Appendix A

This document provides a collection of worksheets designed to assist users in configuring and managing an ETOS V5B system. The worksheets are intended to be used as a record for system options and settings. Users are advised to make copies of these blank forms, fill them out in pencil as the system is built or modified, and keep them stored with their Manager's Guide for reference.

The document contains the following specific worksheets:

• System/Program Worksheets: Tables for recording stand-alone OS/8 and Virtual OS/8 programs and their lengths.
• Configuration Worksheets: Tables for tracking binary patches, configuration table entries (memory, terminal, filler, interrupts), and V5B-specific table settings (scheduler, buffers, command lines).
• Directory and Storage Worksheets: Forms for managing Master File Directories (UFDs), library files, operator files, user accounts, and overall disk block usage and storage area details.
• Device Handler Worksheets: Tables for tracking stand-alone and Virtual OS/8 device handler status and SET options.
• Procedural and Patch Worksheets: Forms for recording optional patches, mandatory patches, and site-specific start-up and shut-down procedures.
• User Feedback: A standard form for users to provide comments regarding the clarity and effectiveness of the manual.
