Appendix A

This document serves as an appendix (Appendix A) containing a collection of worksheets for ETOS V5B system management. These forms are intended to be copied and used by system administrators to record and track various system configurations and settings as they build and maintain their environments.

The document includes the following specific worksheets:

• Program and Storage Worksheets: Tables to record program lengths and disk storage area allocations (OS/8, Virtual OS/8, and stand-alone environments).
• Configuration Worksheets: Forms for recording binary patches, system configuration tables, and specific V5B table values.
• Directory and Account Worksheets: Templates for managing Master File Directories (UFDs), libraries, operator accounts, and individual user accounts.
• Operational Worksheets: Forms for tracking mandatory/optional patches, start-up/shut-down procedures, and device handler status.

The instructions emphasize that users should make copies of these blank forms, use pencil to allow for updates when configurations change, and keep these records alongside their primary Manager's Guide. The final page provides a user comment form for feedback on the documentation.
