- Purchase Drivers: A rating of the importance of various factors in the purchase decision (e.g., price, performance, expandability, vendor reputation, ease of use, video quality).
- User Information: Contact details (name, phone, company, address) and primary job function.
- Qualitative Feedback: Open-ended questions asking about the user's likes, dislikes, suggested changes for the workstation, and feedback on the product documentation.
The completed questionnaire is intended to be returned to Digital Equipment Corporation via a prepaid business reply mail service.
